One of the leading US Senate proposals to introduce a federal carbon trading scheme has drawn criticism from a lobby group of companies that is involved in the buying and selling of emissions credits. The International Emissions Trading Association Ieta ; said in a position paper yesterday that while it broadly supports the emissions trading proposal sponsored by Independent Senator Joe Lieberman and Republican Senator John Warner, it recommends several changes in key provisions. The group urged the Senate's environment committee to revise the bill's plans for a `central bank' type agency to intervene in the market if prices for carbon credits go too high. "Price management is the option least favourable to Ieta, since it is the most interventionist and would essentially eliminate most benefits arising from a market system, " Ieta said. Instead, the lobby group said it favoured a "robust supply of offsets" to maintain supply and demand balance for international carbon credits, and to ensure a liquid market. Offsets are credits for emissions reductions achieved through projects such as reforestation efforts, with which emitters may compensate for the amount by which they exceed their greenhouse gas targets. Ieta welcomed a provision in the bill that would allow international offsets to account for 15 per cent of a company's compliance shortfall under federal carbon caps. But the group urged US lawmakers to ensure that offsets would be of suitably `high quality' and meet standards such as those used for credits issued through the UNregulated clean development mechanism. Verified international carbon credits such as certified emissions reductions CERs ; generated through the Kyoto protocol can be bought by emitters in industrialised countries in order to offset domestic emissions. The import of these credits is regulated in Europe's emissions trading scheme, and in many EU countries there is a limit on how many offset credits that can be used for compliance of 10 per cent of each installation's allocation. Ieta said access to offsets "across broad geographic boundaries" would be the most effective method of reducing the costs of complying with a federal US cap-and-trade scheme, meaning companies should be allowed to purchase credits from emissions reduction projects located anywhere in the world.

Behcet's syndrome is a systemic inflamma tory disorder with unknown etiology. Vascular endothelial dysfunction is one of the prominent features of the disease. We previously demonstrated the possible involvement of proinflammatory cytokines [tumor necrosis factor TNF ; -, soluble interleukin-2 receptor sIL-2R ; , interleukin IL ; -6 and IL-8], nitric oxide NO ; and adrenomedullin in the etiopathogenesis of Behcet's syndrome. Since VEGF expression is induced by these cytokines and VEGF itself is a potent stimulator of NO production with endothelial cell effects, this study aimed to investigate whether VEGF was affected during the course of Behcet's syn drome. We also assessed the possible involvement of VEGF in ocular Behcet's syndrome or in disease activity. Methods: This multicenter case-control study included a total of 39 patients with active n 22 ; or inactive n 17 ; Behcet's syndrome mean age, 38.1 10.4 years; 21 men and 18 women ; satisfying International Study Group criteria, and 15 healthy hospitalbased control volunteers mean age, 39.2 9.3 years; eight men and seven women ; matched for age and gender from a similar ethnic background. Patients were examined by a dermatologist and an ophthalmologist with an interest in Behcet's syndrome. Plasma VEGF concentrations were measured using a newly established enzyme-linked immunosorbent assay. Clinical findings and acute-phase reactant parameters such as erythrocyte sedimentation rate -antitrypsin, 2 -macroglobulin, and neutrophil count were used to classify the disease in Behcet's patients as active or inactive. The Wilcoxon test or the Mann-Whitney U-test was used for statistical analysis as indicated and the results were expressed as mean SD, with range. Results: The mean plasma VEGF level in patients with Behcet's syndrome 291.9 97.1 pg mL; range 121-532 pg mL ; was higher than that in control subjects 103.0 43.6 pg mL; range 25-187 pg mL ; and the difference was significant P 0.001 ; . Patients with active disease had significantly P 0.001 ; higher VEGF levels than patients with inactive disease 347.6 87.1 vs. 219.9 51.6 pg mL ; . addition, ocular Behcet's patients n 23 ; had higher VEGF levels 315.7 92.1 pg mL ; than nonocular patients n 16, 257.8 96.6 pg mL ; and the difference was of borderline significance P 0.041 ; . The levels of all acute-phase reactant parameters were significantly higher in the active stage than in the inactive stage for each, P 0.01 ; or in control subjects for each, P 0.001 ; . Conclusions: VEGF may participate in the course of Behcet's syndrome, especially in the active stage, and elevated levels of VEGF may be an additional risk factor for the development of ocular disease, contributing to poor visual outcome. 177. Randomised, controlled trial of men with primary osteoporosis or osteoporosis related to hypogonadism. There were increases in the BMD of the spine 7.1% ; , femoral neck 2.5% ; and femoral trochanter 4.4% ; . Height loss was prevented by alendronate and there was also a reduction in the incidence of radiographic vertebral fractures.9 Glucocorticoid-induced osteoporosis Bisphosphonates may prevent glucocorticoid-induced osteoblast and osteocyte apoptosis. Two large multicentre trials of etidronate and alendronate in glucocorticoid-induced osteoporosis show that both are effective at increasing spinal BMD at 12 months.10, 11 Only alendronate increased BMD in the femoral neck, but both drugs significantly increased the BMD in the femoral trochanter. There was a non-significant trend for a reduction in vertebral fractures in postmenopausal women in both studies. R8sedronate increases BMD.12 A daily dose of 5 mg has also recently been reported to significantly decrease vertebral fractures by 70% in patients with glucocorticoid-induced osteoporosis.13 In a preliminary uncontrolled study in glucocorticoid-induced osteoporosis, pamidronate significantly increased spinal BMD by 4.7% at one year. There was no change in hip BMD. By comparison, calcitriol therapy prevents spinal bone loss in glucocorticoid-induced osteoporosis. However, it does not prevent bone loss from the femoral neck, nor are there antifracture efficacy data. Prevention of postmenopausal bone loss Women with low BMD who are either intolerant of or unwilling to accept therapy with either oestrogen or raloxifene see `New drugs' Aust Prescr 1999; 22: 967 ; would be suitable for treatment with a bisphosphonate. Bisphosphonates have been shown to prevent postmenopausal bone loss. In early postmenopausal women without osteoporosis, alendronate prevents bone loss at all sites except the forearm. In similar women risedronage increases the BMD of the spine and hip, and decreases bone turnover.14 In studies of these drugs fracture reduction was not an end-point. Future directions Current clinical studies are examining the comparison of intravenous and oral dosing, and the optimal frequency and duration of oral dosing. In the future there may be additional indications for this class of drugs as they affect metabolic pathways throughout the body, not just in bone cells and spironolactone.
Online purchasing of health care goods and services is expected to skyrocket over the next few years, but Internet merchants will have a tough time convincing consumers that online shopping is really easier than going to the store. A study released in January by Internet research firm Jupiter Communications predicts that online spending for health goods and services will climb from $200 million in 1999 to $9.8 billion by 2004. This explosive growth could be even faster, the study shows, but most consumers still find buying in traditional stores more to their liking. Nearly one-half of the 1, 667 consumers in the Jupiter survey said they preferred shopping in a traditional store where they can get personal attention, low prices and convenience in returning goods. "Consumers say it is easier to shop off line right now, " Claudine Singer, a Jupiter analyst, told the Associated Press. The Jupiter study found that about 45 percent of online health spending in 2002--$4.4 billion-- will be for prescription drugs, up from $30 million in 1999.

INTRODUCTION: Iron deficiency anemia IDA ; in infancy alters several brain functions, including neurotransmission systems, myelination and neuronal metabolism. Cognitive, emotional and motor behavioral domains are affected in IDA infants. Thus it is plausible that alterations in sleep development might be related. Sleep spindles are key features of the normal progression of sleep maturation and appear related to the consolidation of synaptic circuits and memory processing. Therefore, an alteration in sleep spindles organization could be a contributing factor for altered developmental outcome in these infants. OBJECTIVE: To determine if sleep spindles in IDA infants show different patterns compared with controls DESIGN METHODS: Polysomnographic recordings were performed in otherwise healthy 6 month-old infants who had IDA n 26 ; or were nonanemic controls, n 18 ; . Sleep wake states were determined visually, and sleep spindles were detected in central derivations by an automated procedure and then edited for visual confirmation. For each infant the following parameters were determined for NREM stages II and SWS: sleep spindles index number min ; , and mean duration of sleep spindles and of inter-spindles intervals. RESULTS: Total sleep duration and the duration of NREM stages II and SWS were similar in both infant groups. Compared with controls, IDA infants had lower sleep spindles index p 0.01 for stage II, and p 0.04 for SWS ; , and a suggestive tendency for shorter spindles duration and longer inter-spindles intervals. CONCLUSIONS: Our results show that IDA in infancy alters the organization of sleep spindles, indicating that iron is essential for the underlying mechanisms that assure neurofunctional maturation. Since sleep spindles appear to be involved in memory processing, we speculate that these NREM characteristics may contribute to reduced cognitive performance in IDA infants. The high incidence of hip fractures in elderly patients with AD may be attributed to frequent falls4 and osteoporosis due to 25-OHD deficiency with compensatory hyperparathyroidism as suggested in the present study. Serum levels of 25-OHD and PTH and body mass index were found to correlate with BMD in patients with AD. In the present study, the number of falls during the follow-up period was similar and reduced compared with baseline values in the 2 groups, indicating that ergocalciferol may have prevented falls in both groups. A recent study suggested that vitamin D supplementation reduces the risk of falls among ambulatory or institutionalized older individuals.26 This implies that the frequency of fractures due to falls in patients with AD is related to hypovitaminosis D. Despite the reduced incidence of falls and normalization of 25-OHD, incidence of nonvertebral fractures was higher in the control group. Indeed, RRs for hip and nonvertebral fractures were low in the risedronate group compared with the control group. Also, the RR of the risedronate group in the present study 0.26 ; was low compared with that reported in a recent metaanalysis of 7 trials 0.73 ; on the effect of risedronate for osteoporotic fractures.17 This difference may be explained by a larger increase in BMD in the present study: the average values of BMD increased by 4.1% in the risedronate group over 18 months. This may reflect a synergistic effect of risedronate and ergocalciferol. The hip fracture rate in the control group was calculated as 57 per 1000 patient-years. The rate of hip fracture in an elderly reference population between ages 70 and 79 years was reported to be 6.6 per 1000 patientyears.27 Although the mean age of our AD subjects 77.7 years ; was within this range, the fracture rate in the pres.

Over the past few years, the Heinz Family Philanthropies has undertaken a number of groundbreaking studies assisting Governors and state legislators address the problem of skyrocketing prescription drug costs. At the request of Governor Kempthorne and the state legislature, the foundation provided the State of Idaho State ; with a grant to hire Mercer Government Human Services Consulting Mercer ; to perform a comprehensive analysis incorporating both a detailed examination of the State Medicaid pharmacy program and a concurrent study of the efficiency of provider practices within the State. This report exclusively presents the findings from the detailed analysis of the State's Medicaid pharmacy program completed by members of Mercer's Government Pharmaceutical Practice. The situation in Idaho is not vastly different from what we have seen in other states -- ever increasing costs that are significantly impacting the State's Medicaid budget. Solutions to address these increasing costs do exist, but also present a series of tough choices and decisions for legislators and policy makers. Our pharmacy consultant, Mercer, has identified several potential solutions, however, it should be recognized that certain programs overlap and affect similar therapeutic categories. Therefore, the cumulative savings may be mitigated, depending upon the programs that are implemented. Medicaid expenditures for fee-for-service, outpatient prescription drugs have grown rapidly in recent years, motivating states to use various cost and utilization controls. In more recent years, this rise in prescription drug expenditures has also been accompanied by a decline in state revenue collections due to the economic situation. As a result, policymakers have been forced to consider how to contain the State budget, and therefore, Medicaid pharmacy programs have become a major cost containment target. Given the magnitude of budget deficits, many states are focused primarily on cost-containment strategies that produce immediate savings, such as reimbursement cuts, reductions in, for example, risedronate sodium tablets.
