Use in Pregnancy Category C ; There was no evidence of embryotoxicity or teratogenicity in rats and rabbits treated orally with mesalazine during the period of organogenesis at respective doses of up to 320 and 495 mg kg day representing about the same, and nearly twice, the maximal recommended clinical dose of Salofalk granules on a body surface area basis. Human data on use during pregnancy are limited. There is a theoretical risk that, in common with other anti-inflammatory agents, mesalazine may produce premature closure of the ductus arteriosus and may, if given at term, prolong labour and delay parturition. The administration of aspirin acetylsalicylic acid ; increases the bleeding tendency in the neonate and the mother. A similar pathological bleeding tendency in the neonate and the mother is not expected with mesalazine. However, mesalazine is a salicylate and, in general, should be used only in the first trimester if strictly indicated. If possible, women should postpone conception until a phase in which minimal drug treatment is required. The individual disease activity permitting, treatment should be discontinued in the last 2-4 weeks of pregnancy. Use in Lactation In rats, there were no adverse effects on dams or offspring from oral administration of mesalazine during late gestation and throughout lactation at doses up to 320 mg kg day which is about the same as the maximal recommended clinical dose of Salofalk granules on a body surface area basis. There has been a report of a patient receiving mesalazine suppositories during the lactation period. Twelve hours after the initial dose, the infant developed watery diarrhoea that disappeared on discontinuation of the mesalazine therapy but reappeared on rechallenge. There have been reports of mesalazine and of its metabolite N-acetyl-5-ASA found in breast milk. There is no experience with SALOFALK granules in lactating women. Salofalk should not be used during lactation unless the likely benefit of treatment outweighs the potential hazard. Use in Children SALOFALK should not be used in children 12 years old and under, as there is no experience with this age group. A case of myxedematous ascites is described in which concern arose because of concurrent elevation of serum tumor markers, in particular CA 125, to levels strongly associated with ovarian carcinoma. A 74-yr old woman presented in July 1995 with a 2-yr history of progressive weakness. Over the preceding 3 months she had developed abdominal distension, anorexia, and constipation. She had also noted cold intolerance and deepening of her voice. Previously she had taken thyroxine; however, for unexplained reasons, it had been ceased 2 yr before admission. There was no history of hyperthyroidism, neck surgery, irradiation, or of radio-iodine administration. She smoked 20 cigarettes day and drank approximately 40 g ethanol day. There were no other past illnesses, operations, or medications. She appeared cachectic and spoke with a hoarse voice. Her weight was 46 kg, height 163 cm, temperature 36.2 C, pulse 6O minute, and blood pressure 145 70 mmHg. The thyroid was not palpable. She had tense ascites and a right pleural effusion, together with peripheral and periorbital edema. There was a trace of protein in the urine, which was otherwise normal. The jugular venous pressure was elevated 6 cm, but the heart sounds were normal and easily audible. The liver was not palpable. No pelvic masses were palpable. Proximal weakness and some slowing of cognitive functions were present. Ocular examination was normal. Initial investigations confirmed primary hypothyroidism with a thyrotropin TSH ; level of 50.1 mIU L [reference range RR ; 0.3-4.01 and free thyroxine level of 4.0 pmol L RR 12-21 ; . Computerized tomography of abdomen confirmed ascites, but liver, spleen, pancreas, and renal tracts were normal. The ovaries were not enlarged, and no pelvic masses were seen. Duplex doppler scan of the portal vein showed normal flow. An echocardiogram showed no evidence of pericardial effusion and was otherwise normal. Chest x-ray demonstrated the right pleural effusion. Following 6 weeks of thyroxine replacement 25 kg daily ; , the patient was readmitted to hospital with increasing ascites and peripheral edema. Spironolactone and frusejide were commenced. Thyroxine dose was increased to 50 pg and then to 100 pg day over 8 weeks. Over these 8 weeks, dyspnea, peripheral edema, and ascites resolved, and serum tumor markers, including CA 125, fell towards their reference ranges. Spironolactone and fluid restriction were stopped, and frusemide dosage tapered. The patient was maintained on thyroxine, 100 pg day and remained well with no evidence of recurrent ascites or edema, nor of any malignancy at 22 weeks of follow-up. It has been well established that hypothyroidism is associated with elevation of lactic dehydrogenase and elevations in serum CEA, CA 125, AFP, and CA 15-3 of 1.2-1.7 times the mean of normal subjects 1 ; . Our patient had elevation of CA 125, CEA, and AFP to 19.5, 2.6, and 1.9 times the upper limit of normal.
